Faulty weld ruptures
A pulp mill facility in Prince George experienced a weld failure that occurred on a steam distribution box of its heat exchanger. The distribution box allows for a more even distribution of steam into the shell side of the heat exchanger which is used to heat green liquor.
The longitudinal weld on the box ruptured releasing steam to the atmosphere.
The investigation showed that a pressure excursion and an inadequate weld led to the incident. The weld did not have full penetration as required by the original design.
No injuries or fatalities were reported as a result of this incident.
