Atmospheric Boiler Overheated Due to Lack of Maintenance

Incident overview

A boiler, which had not been stripped and cleaned for over five years, overheated and led to a fire within the unit. Investigators found that the boiler had become plugged with soot over the years. Upon ignition, flames spilled from the combustion chamber and ignited gas on the right side of the boiler. A flame melted the control panel and ignition control box.
