A boiler in a commercial space had a high-pressure delayed ignition event that dislodged the boiler vent. The boiler continued to operate, letting exhaust gas with carbon monoxide into the building. This likely occurred because a broken regulator let too much gas into the boiler, causing a strong ignition that dislodged the vent. Without CO detectors, there was no early warning of the carbon monoxide leak.
