Incident Overview

A natural gas furnace’s secondary heat exchanger failed, resulting in high production of CO. Re-circulation of flue gas through air intake grills and building openings allowed CO to enter the home. Lack of maintenance also contributed to the incident by allowing the condensate drain to plug up.
