Incident Overview
Two boilers and their natural draft venting systems were discovered to be corroded and damaged beyond repair. Investigation found that a bypass mixing valve was left in the off position, causing the inlet water temperature to be too low. This resulted in corrosive condensate forming in the boiler combustion chamber and causing corrosion damage to the venting system.
