Delayed Ignition Causes Damage to Boiler and Venting

Incident overview

An ignitor was incorrectly installed on a boiler, which contributed to its delayed ignition. The boiler, housed in the mechanical room of a multi-residential building, was damaged in its internal combustion chamber. The front cover blew off along with the venting fitting at the top of the boiler. No safety mechanisms were in place to prevent the boiler from operating with the dislodged venting indoors.
