Incident overview

During an emergency brake test procedure known as “unintended motion” the emergency brakes failed to deploy. The car continued to ascend until its counterweight collided with the buffer and the car struck the overhead beam. The impact caused moderate damage to the elevator components, including the elevator sling, the overhead hitch beam and floor tiles.
