Incident overview

As a construction hoist traveled down, the cab began to sway and rock excessively, then struck the tower causing significant damage to a 130’ tower section. Other components of the cab were also scraping the tower as the cab traveled up and down. The likely cause is a faulty bearing in one of the guide rollers.
