Incident overview
A radiant heater caught fire due to a gas leak. The hose between the gas valve and burner was installed with a sharp 90-degree bend at the gas valve end. The strain on the hose caused it to crack and led to a gas leak which ignited and burnt the control wiring and ignition module. No one was injured.
