Incident Overview
A cement kiln used for manufacturing cement products was ignited by a natural gas pilot burner system. The automatic pilot had stopped working. Workers bypassed the safety switches and tried to manually operate the pilot, resulting in gas escaping through the open scanner position hole and igniting.
