Incident overview

A hot water boiler in a rooftop mechanical room was emitting high levels of CO out to the rooftop, where a nearby rooftop air handler fan pulled the CO into the building, setting off alarms. The likely cause of the hazardous CO levels was a faulty gas valve that led to improper combustion.
