Incident Overview
A natural gas boiler in a residential home developed a hole in the burner after 10 years of operation without cleaning or service. With a hole in the burner mesh, flames entered the premix area in the fan coupler, creating a hole in the appliance where flames escaped. Residents smelled smoke around 4:00 AM and found flames coming out of the top of the boiler. They were treated for smoke inhalation and burns on their hands.
