Incident overview
In a large residential home, a gas-fired, condensing heating boiler was used as the main source of heat, even though it was undersized to serve the large property. The boiler was installed without the appropriate permit and, upon investigation after the incident, was found that it did not have the correctly sized venting system, code compliant vent terminations including separation needed in the air intake and vent outlet and had incorrect air fuel adjustment at low fire. Over the course of two years, six occupants were exposed to carbon monoxide.
