Incident Overview

A free-standing, natural gas fireplace was incorrectly installed inside of an existing wood-burning fireplace as an insert in a rental home. The installation did not follow manufacturer’s instructions and was unable to vent the products of combustion correctly. The incorrect venting along with the damaged fireplace glass panels allowed carbon monoxide to leak into the home as the fireplace was not sealed. Two adults and two children were exposed to carbon monoxide.
