Incident overview
An incorrectly installed direct vent termination on a free‑standing gas fireplace allowed combustion products to recirculate into the appliance, creating an unsafe condition that generated extremely high levels of carbon monoxide. These gases entered the living space, leading the two short-term vacation renters who were staying there to seek medical attention for suspected carbon monoxide exposure.
