Incident overview

While charging a Tesla at a fast-charging station, an uncertified third-party adapter failed, causing a high-voltage arc flash that injured the vehicle owner and caused major damage to the vehicle, charger, and adapter. The short circuit originated within the adapter, which was not approved for use in Canada and lacked a recognized certification mark. Watch the video.
