Incident overview
A small fire occurred in an elevator’s closet. The braking transistor shorted causing the braking resistors to overheat. During investigation, it was found that the controller had no external thermal switch to monitor the braking resistors’ temperature. It’s likely that a monitoring circuit could have prevented the overheating of the resistors.
