First responders and patient entered a hydraulic elevator, which had undergone major alterations in 2011, and were entrapped by a tripped over-speed valve. Causes and contributing factors that caused the elevator to stop have been attributed to a combination of improper over speed valve setting and compressions from CPR being performed during the journey.
