Incident Overview

A worker tried sending a dumbwaiter down when they heard unusual noises in the hoistway. While trying to dislodge a basket that was stuck in the hoistway, the worker fell down the shaft sustaining lacerations to the face, bruising to the back, shoulder, and arms. The lack of approved safety components and unregistered design contributed to the accident.
