Incident Overview
Three operators at a gas processing plant were asked to remove two inlet filter vessels due to potential blockage. The workers failed to confirm that there was zero pressure in the vessel. Once the last clamp was loosened, the hatch opened, releasing the filters, ice, and hydrate from the vessel. One worker sustained bruising to the right-side torso and leg.
