New and Altered Devices
To book a physical assessment (inspection) for your elevating device, please have your elevating devices contractor contact the safety officer assigned to your area and submit the applicable forms.

Failure to Submit Applicable Forms
Please note: failure to submit the applicable forms or submission of incomplete forms may result in the inspection being failed.
The contractor is responsible for ensuring that all the testing equipment used to demonstrate compliance during the acceptance test is available on site.
Non-compliances issued prior to a final acceptance inspection must be declared before requesting a final acceptance.
About Failed Inspections
Your inspection may be failed if testing equipment is unavailable to demonstrate compliance during an acceptance test. It is the responsibility of the contractor to ensure the appropriate testing equipment is available during the inspection.
Similarly, inspections may not pass if repairs or corrections cannot be completed within a reasonable time frame, at safety officer discretion. The repair or correction will be noted on the Certificate of Inspection.
Non-compliances include a high hazard condition.
After a Failed Inspection
Failed inspections will result in the need for a re-inspection, which have a minimum two-week wait for re-inspection. Any applicable travel costs (mileage, travel time, accommodation, meals), will be billed to subsequent re-inspections in addition to the re-inspection fee(s).
Non-compliances related to a failed inspection must be corrected and declared prior to requesting a re-inspection.
The same procedure for booking an inspection should be followed to book a re-inspection.
Cancellations
Cancellations must be received at least two full business days* before your scheduled inspection and should be made directly to the safety officer. Cancellations outside of this timeframe will be considered a failed inspection and full inspection fees, including applicable travel costs (e.g., travel time, accommodation, flights, meals, car rental) will apply. Any costs related to booked airfare and accommodations will be charged regardless of when notice was given.
E.g., An inspection is booked for Friday; the safety officer must be notified of the cancellation no later than the Tuesday prior. Business days are exclusive of statutory holidays.
Booking after any cancellation may result in a minimum one-week wait time.
Overtime
If you are requesting an overtime inspection, the request for overtime inspection form must be submitted.
In addition to inspection time, overtime hour billing may include administrative and travel time.
Removal/Return to Service
A request to remove a unit to service does not incur additional cost.
A request to return a unit from service may be billed a minimum of two hours, see “special inspection” under the fee schedule.
